(hereinafter referred to as the Enterprise) to process my personal data in card files and/or through the information and telecommunication system of the Enterprise's personnel database for the purposes of personnel record management, preparation of statistical, administrative and other personnel-related information in accordance with legislative requirements, as well as internal documents of the Enterprise concerning the implementation of rights and obligations defined by legislation in the field of labor relations and social protection.
By this consent, I also grant the Enterprise the right to provide my personal data to:
